Please note: The collections sites do not have the required drug screening paperwork. Please be sure to have your employee(s) take the paperwork that was provided to you as well as a picture ID.

Please refer to your chain of custody (multilayered form). Your designated lab will either be in the top left hand corner (LabCorp) or in the top right hand corner (Quest Diagnostics).
